PARTNERSHIPS. Three Steps Home partners with community-based organizations and healthcare providers to deliver coordinated, housing-focused care for individuals experiencing homelessness and complex medical needs. These partnerships are essential to bridging a critical system gap between hospital discharge and stable housing.
Key partners—including Peter & Paul Community Services, Places for People, Novus Health, and LIV Recovery—provide access to housing navigation, behavioral health services, and recovery support, ensuring clients receive comprehensive, wraparound care beyond their medical stabilization.
The program is also designed to integrate with regional hospital systems such as BJC HealthCare, Mercy, and SSM Health to support safe, appropriate discharge planning. While formal referral pathways are in development, the model is intentionally structured to align with hospital workflows and the broader Continuum of Care.
Three Steps Home maintains an open, collaborative partnership approach and will actively engage with any organization whose mission and services align with its commitment to holistic care, housing stability, and improved health outcomes.
Together, these partnerships ensure continuity of care, improve housing placement outcomes, reduce avoidable hospital utilization, and strengthen coordination across the healthcare and homeless response systems.
